Supplemental Applications:

• Supplemental Application A: Claims or Incidents -- Used only if you have more than one claim.
• Supplemental Application B: Mergers or Acquisitions -- Required if you have merged with another agency or if you have purchased one within the past three (3) years.
• Supplemental Application C: Cluster Arrangement -- Required if you are part of a Cluster, call for an explanation if you need to.
• Supplemental Application D: Real Estate Operations -- Safeco will provide coverage for some Real Estate Operations. If you need this coverage, complete the application.
• Supplemental Application E: Aviation -- Required if you place 5% or more of your total agency premium volume in this market.
• Supplemental Application F: Professional Employer Organizations -- Required if you provide these services to your clients.

• Insurance Professionals Errors & Omission Liability: Supplemental Carrier Information
• Additional Personnel Form -- Required if you are adding Personnel either mid-term or at renewal. Kentucky Agents, please complete one for every agent that needs a 99-1 filed.
• Insurance Professionals Errors & Omission Liability: Independent Contractor Information -- Required if you have any employee/producer that will have their wages/commission reported to the IRS by way of a 1099 at the end of the year. If you have an "IC" that places less then 75% of THEIR volume through your agency, then they are considered "NON-Exclusive" and Safeco will charge an additional premium for them. Be sure to know what the IC's total volume is and how much they put through your agency.
• Employers Practice Liability Endorsement -- (Application & Instructions) Optional coverage. It is a supplemental coverage on the policy, however it will have separate limits and deductible. This cannot be quoted mid-term or after the policy is put into place as it is a "supplemental provision" to the policy.
